A request to be allowed to return to work must be received within 60 days of the claim administrator's receipt of the Physician's Return-to Work and voucher Report (Form DWC-AD 10133.36). The SJDV voucher is worth 6000 dollars for any level permanently disabled (1 1 % to 100 percent) and is available to attend an California private school, or other institution which is on the state's approved list of training providers. 

The SJDV is also able to be used to cover licensing or certification fees and tests as well as to purchase the tools needed for a course of instruction or to purchase computer equipment in the range of $1,000 and to pay up to $500 in other costs. A maximum of 10 percent, or $600, can be used to purchase assistance from a certified vocational or placement counselor. 

Not more than 10% of the value of the voucher is used to pay for vocational or return-to-work counseling. In the event of injuries occurring after January. 1st 2013, the SJDV voucher must be returned in 20 days after the date of offering regular or modified work. The work must pay not less than 85 percent of employee's earnings during the period of injuryand is likely to last at minimum 12 months. 

For injuries that occurred after January. 1st of 2013 If the employer offers an offer for regular or modified work within 60 calendar days of the claim administrator's receipt of the Physician's Return to Work & Voucher Report, and the offer is in line with certain conditions and you refuse the joboffer, you're no longer qualified for the voucher. Modified or alternate work has to meet the following requirements:

• You're able to carry out the fundamental tasks of the job

• The position is a full-time job that lasts at minimum 12 months

• The position offers the opportunity to earn wages and compensation which is at or near 85 percent of the amount which you are paid when you suffered the injury.

• The work is within commuting distance from your home at the moment of the injury.
